    Best explanation:
    Hangup in the machine turning the extractor groove and rim.
    The real question is how it got through inspection.
    Note the lack of a proper bevel in the extractor groove.

    Obviously operator-error because who sees a round like that and still loads it and fires it? Darwin Award contestant, that's who.

    If the case was machined all "ribbed for your pleasure" like that BEFORE it was fired, why would they load it in a weapon they cared about and fire it with their own hands/face nearby in a training class?

    It was training class that was fired by one of two students that were in the class and likely they did not know any better.
    I had a CWP class today and two people shot 9 mm in the same lane. Both shot Federal ammo, but one set of 50 cases was distinctly stained/discolored, with one case in particular extremely distorted. I believe that these were shot in a S&W M&P9 M2.0 9mm with 5-inch Barrel.

    What is the likely cause for this case distortion?

    Something to keep in mind when teaching a beginning class to remind the students to pay attention to what they are loading. The reaction from most of us was the round was already distorted prior to loading. Someone else pinned down the likely cause of it.
