Yep... plus plain luck. Even with FMJ, one solid hit in the head or heart or major vessel and you're (probably) done. And I wouldn't be surprised if the old lady was only loaded with .38s, since .357s are no fun with "atha-ritis" like mine. Or maybe she's just tougher than me. Does appear she got off the first shot, though, which I'll bet threw the perp off his game
